Appointments

All services are by appointment only.  Please call 773-955-9643 to schedule an appointment.

Fee for Service

Our services are provided on a fee-for-service basis. This means that the entire fee for the treatment session is due at the time of the visit.  If your insurance covers the treatment, we ask that you pay at the time of service, and we will assist you in obtaining reimbursement from your insurance company.

We accept cash, checks and most major credit cards (Visa, Master Card, Discover and American Express).

Cancellation Policy

We ask that you notify us at least 24 hours in advance if you must cancel your appointment.  We must charge the full fee if you fail to come for your appointment or if you cancel less than 24 hours in advance.

As a courtesy, we make reminder calls 24-48 hours prior to your appointment.  However, we are not always able to reach people.  It is still your responsibility to remember your appointment.
